Vax Verify
Individuals can request access to their immunization records online through the Illinois Department of Public Health’s (IDPH) Vax Verify portal.Â

Individuals who need additional assistance are welcome to come into WCHD and use the kiosks on the 4th floor for computer and internet access for the purpose of requesting their immunization records. WCHD staff may assist the individual with their Vax Verify registration process if individuals are needs assistance with the technology.
Medical Records From WCHD
Individuals who received an immunization through the Winnebago County Health Department and are unable to utilize Vax Verify, may come in person to the 4th floor registration desk to request a copy of their immunization records. Only immunizations received through WCHD will be available through this format. Individuals will need to fill out a Release of Information Form in person with the staff on the 4th floor and provide a photo ID.Â
