The Health Of Winnebago County

Starting February 13 through March 8, Winnebago County residents are asked to share information on what impacts their health to identify needs and gaps in health and shape a plan to address the priority health needs of our community. All Winnebago County residents 18 and up are encouraged to participate.

Community

The Winnebago County Health Department works with residents and our partners every few years to assess the health of our community. Input from residents is part of a process of collecting information on the status of health in Winnebago County known as a Community Health Assessment.

Data

The data from this assessment process along with other sources of data is used to identify and prioritize health needs and gaps in our community. Further exploration of those needs and gaps is done with focus groups of community members.

Plan

A plan is developed for how to address the prioritized health needs/gaps. This is called the Community Health Improvement Plan (CHIP)/Illinois Project for Local Assessment of Needs (IPLAN). The Winnebago County Health Department works with residents and community partners to address the priorities.

Community participation is critical to developing a plan that works to improve health and quality of life. By participating in our Community Health Assessment, you are helping to shape the health of Winnebago County!

Community Health Assessment: Community Partner Toolkit

Community Partners and local organizations are in a position to assist Winnebago County residents in sharing what impacts their health. Some ways to assist include:

  • Asking your staff and clients to share what impacts their health by submitting the online questionnaire (send a note to your staff and clients via email or inform them at office visits)
  • Promoting the completion of the questionnaire by hanging posters or sharing on social media
  • Assisting your clients with completing the questionnaire. You can even host WCHD staff at your facility to assist your clients.

Resources, such as template emails and social media messages, are available for your agency/organization to utilize in the partner toolkit.
