If you would like WCHD to attend your event (such as setting up an informational table to engage with your attendees) complete the form below at least 3 weeks prior to your event. Completing the form will help us better understand your event’s needs and how we can best contribute.
Due to limited resources for outreach activities WCHD cannot attend many events. Each event will be evaluated based on WCHD’s ability to improve health outcomes in our community through participation and availability of WCHD resources.
