Annual Report
Each year, WCHD issues a report that summarizes the activities of the department over the past year based on the public health domains. This report also contains information about the number of staff and the annual budget for the department.
IPLAN
As a certified local and accredited health department, WCHD conducts a community health assessment (CHA) and develops a community health improvement plan (CHIP), Â also known as the Illinois Project for Local Assessment of Needs (IPLAN).

The MAPP 2.0 Framework (Mobilizing for Action through Planning and Partnerships) was used to develop the IPLAN for Winnebago County. As part of MAPP 2.0, the following assessments were done:
- Community Status Assessment – Quantitative data on key health indicators including demographics
- Community Partner Assessment – Assessment of capacity and strengths of community partners in addressing the health of the community
- Community Context Assessment – Input from the community on health on health and what impacts health
