The University of Illinois College of Medicine Rockford invites you to join TEAM (Text for Education and Management)!

Do you have Type 2 Diabetes and an A1c over 7%? The University of Illinois College of Medicine Rockford invites you to join TEAM (Text for Education and Management)!

Participants in Winnebago, Boone, Ogle and Stephenson counties over age 18 can receive text message support and reminders designed to improve blood sugar levels and support diabetes self-management as part of a research study.
